The Smith County Clerk’s office has records for deaths that occurred outside the city limits of Tyler.

Deaths Occurring INSIDE the City Limits

If the death occurred inside the city limits of Tyler you may contact the Northeast Texas Public Health District at (903) 535-0030 or (903) 535-0031.

If the death occurred in another county in Texas, contact the County Clerk’s office in the county where the death occurred, or the State Bureau of Vital Statistics in Austin at (888) 963-7111.

You MUST be a Qualified Applicant to obtain a birth or death record.

Death records are closed to the public for 25 years after the death. Certified copies can be obtained by an immediate family member or his/her legal representative. You must present your valid state or government issued photo ID.

Acceptable Forms of ID to obtain a Death Certificate:

The fee to obtain a death certificate is $21 for the first certified death certificate and $4 for each additional certificate requested at the same time.

  • We accept credit/debit cards, cash, cashier’s checks, personal checks (with proper I.D.), and money orders made payable to Karen Phillips, County Clerk.

TO PAY BY DEBIT/CREDIT CARD: Complete the one-time debit/credit card authorization form and return with your original application and a self-addressed stamped envelope.
